Infections of the Vertebral Bone, Discs, Ligaments are more common than previously thought. The most common infection affecting the spine in India is tuberculosis. These infections may start off insidiously with some back pain, but they have the potential to progress, and produce severe structural damage to the spine, and in addition, because of pressure symptoms over the adjacent spinal cord, have the potential to cause paralysis. Thus in a small number of patients, the disease takes a malignant form and has potential to cripple. Fortunately, most of the patients can be treated conservatively, with medicines and rest along with some form of spinal support or bracing. Careful monitoring of the response to therapy is a must throughout the entire course of treatment to monitor for complications of the disease and the medication. Typically the course of medication lasts for 18 to 24 months.
The role of surgery in tuberculosis of the spine is well established. A number of procedures may be carried out, varying from simple drainage of the pus collection, biopsy or sometimes, extensive surgery to correct deformities, and stabilize the spine.
A high index of suspicion, early detection and aggressive initial treatment almost assures a good response to treatment.
Patients must be aware that resistance to antibiotics is increasingly common now and careful monitoring of the disease is absolutely necessary to identify disease resistant organisms and treat them accordingly.
